Daycare Release Forms You Need For Your Home Daycare

As a home daycare provider, you are responsible for ensuring the safety and well-being of the children in your care. Part of this responsibility includes having the proper daycare release forms on hand in case of an emergency. Here are a few essential daycare release forms you should have on file at your home daycare:

Daycare Emergency Contact Form

This form should include the names and contact information of the child’s parents or guardians, as well as any other emergency contacts (e.g., grandparents, aunts/uncles, etc.). It is important to have this information on hand in case you need to reach a parent or guardian in an emergency situation.

Daycare Medical Release Form

This form grants you permission to seek medical attention for the child in your care if necessary. It should include the child’s medical history, allergies, and current medications (if any). This form is essential in case of a medical emergency.

Daycare Behavioural Agreement

This agreement outlines expectations for the child’s behaviour while in your care. It should be signed by both the parent/guardian and the child. Having a behavioural agreement in place will help to prevent behavioural issues from arising during daycare hours.

Daycare Release of Liability

This form releases you from any liability in case of an accident or injury that occurs while the child is under your care. It is important to have this form on file in case of an accident or injury, no matter how minor it may be. 

Daycare Right to Photography Form

The Right to Photography form is a very simple form that gives you permission to take pictures of the children in your care and to use those pictures for promotional purposes. This form is important because it protects you from any legal issues that might arise if you were to take pictures of the children without their parent’s permission. 

Daycare Transportation Agreement Form
